Quality Improvement Current initiatives: Brain Attack - Stroke Response Brain Attack is a project to provide evidence-based, coordinated care for stroke and Transient Ischemic Attack (TIA) patients to ensure optimal outcomes in collaboration with regional partners. The pilot started in February of 2009 and continues to gain traction at regional hospitals. The ultimate goal of this project is to regionally evaluate and implement the best treatment for at risk stroke/TIA patients in the critical first 60 minutes after they arrive at the hospital. Chronic Disease Self-Management Workshops - My Health Matters CHOICE is pleased to offer Stanford University's Chronic Disease Self-Management Program (CDSMP). The My Health matters workshop is a six week course taught by two trained leaders who will create a participative atmosphere focused on skill-building, problem-solving, and creating healthy habits to better manage chronic conditions. Read more here. Emergency Department Consistent Care Program (EDCCP) CHOICE began the pilot phase of EDCCP at Providence St. Peter Hospital in December 2003. This program has three specific goals:
Patients who are referred to this program are identified by the hospital as having a high frequency of Emergency Department (ED) use. Many of the diagnoses are much better, and less expensively, treated outside the ED in a comprehensive primary care practice. EDCCP collaborates among regional hospitals to establish a negotiated plan of care with input from a primary care physician (PCP), program staff, and the patient. The client's care is coordinated across the region among ED and PCPs, with CHOICE providing administrative oversight. |
