ConneXions Sites
Functions and Benefits
ConneXions sites will be doing the vital work of
connecting residents of our region to resources that
can address their needs in a comprehensive and
holistic way. Each agency will decide the level of
participation in ConneXions appropriate for them.
Comprehensive and basic sites will carry out the
day-to-day operations of ConneXions: screening,
assessing, and providing referral and information
services for clients. Referral sites will be able to
conserve staff resources by referring people to
needed resources and tracking services offered. All
ConneXions partners will be expected to update
client records when services are offered
Function: Check if clients have been
screened, assessed and entered into the ConneXions
system. If client is not in system, refer client to
a basic or comprehensive site for screening and
assessment. If client is in system, site can refer
to resources, update client records when services
are offered, and update action plans.

Function: Field phone calls and walk-ins;
work with clients to complete the screening and
assessment process; refer clients to appropriate
services; and generate a client-specific action
plan. Basic sites will help clients apply for the
Basic Food program, and educate clients about
navigating the service delivery system.

Function: Conduct all of the basic
enrollment site functions. Facilitate clients
enrolling into Basic Health, Medicaid, State
Children’s Health Insurance Program, housing, and
prescription assistance programs. Act as liaison
between agencies.

All sites will need the following:
- Computer with High-speed Internet connection,
Windows 2000 Professional or XP Professional, and a recommended minimum
requirement of a Pentium 90 processor, 256MB RAM,
and 40 MB hard drive.
- Netscape 4.7+, Internet
Explorer 5+, or other latest edition of a
leading browser
- Cooperation of internal/onsite
IT staff
